We offer three different forms to accommodate different selling prices for your brochure fundraiser. Download and print the form that matches your sales price, or print the blank form and write in the price that fits your fundraiser.

** Please be advised that our brochure order forms do not include all of the flavors currently available when ordering for a traditional fundraiser. Country Meats is in the process of discontinuing less popular flavors and we want to ensure we will have adequate stock of the flavors your supporters choose to purchase. If there is a particular flavor you are looking for, you may order single flavor or mixed cases to supplement your brochure fundraiser.
